Our Transition Program is designed to help patients ensure a safe transition home. This program helps patients safely return home and ensure that they will not need to be readmitted to the hospital.
It is understood that you won’t immediately regain your strength when you are discharged from a hospital. Patients leaving the hospital may lack the support needed to ensure a successful, safe discharge to go home. Our services are focused on giving you and your loved a period of adjustment upon returning home.
We offer the following Transition services:
- Support for emotional adjustments to lifestyle changes
- Assistance in acquiring medical supplies and equipment
- Assisting in development of the care plan
- Rehabilitation guidance and support
- Preventing re-hospitalization
